Running Club Permission Form for K-2

Parents, We are sorry about the mistake in our running club permission slip.  On the third page, there is a paragraph about choir.  The reason this happened is because Ms. McDaniel allowed us to use her choir permission slip and modify it for running club.  We missed this paragraph.  If you sign the form, you are not committing your child to choir as well.  We apologize for our mistake.  Thank you for understanding.  -The Coaches
